How to correct a factual error in a provider's underlying government record, and separately, how to report a display bug on NPISignal itself.

There are two entirely different kinds of “this is wrong” on NPISignal, and they are fixed in two different places.

The data itself is wrong (address, name, taxonomy, status…)

This has to be fixed at the source

NPISignal does not hold or control any provider's underlying record and cannot edit one directly. Every fact about a provider comes from NPPES, and NPPES is where it has to be corrected. Once corrected there, NPISignal reflects the change automatically on its next import — no request to NPISignal is needed or able to speed that up.

If you noticed the error but are not the provider — for example, an outdated address for a practice you no longer work with — the fastest path is still letting the provider or organization know, since only they (or someone with I&A credentials for that record) can submit the correction.

An LEIE exclusion record is wrong or unclear

The LEIE is maintained entirely by HHS-OIG, independent of NPPES. Questions or disputes about a specific LEIE entry go directly to OIG through their exclusions program, not to NPISignal.

NPISignal displayed something incorrectly

This is different: it means the government's own data says one thing, and NPISignal's page shows something else — a mislabeled field, a broken link, a page that doesn't match what the source file actually contains, or a stale value that should have updated by now. That is a bug in NPISignal, and it is exactly the kind of report worth sending.

  • Include the NPI or page URL where you saw the problem.
  • Describe what NPISignal showed and, if you know it, what the correct value should be according to the source data.
  • If possible, note where you confirmed the correct value (for example, the NPI Registry or the NPPES downloadable file).
