The List of Excluded Individuals/Entities

The LEIE is HHS-OIG’s register of people and organizations excluded from participating in federal healthcare programs. An excluded party cannot be paid by Medicare, Medicaid or any other federal healthcare program — directly or indirectly, and regardless of who employs them.

Who ends up on it

Mandatory and permissive exclusions

Some exclusions are mandatory: OIG is required by statute to impose them, for convictions relating to healthcare fraud, patient abuse or neglect, and certain controlled-substance offences. These carry a minimum five-year period.

Others are permissive: OIG may impose them, for things such as licence revocation or suspension, default on a health education loan, or providing services that are excessive or of unacceptable quality. TheEXCLTYPEcolumn names the specific statutory authority, and one of the most common values, 1128b4, is licence-related rather than fraud-related.

An exclusion ends only when OIG reinstates the party, and reinstatement is not automatic when the period elapses — the party has to apply. Reinstated parties are removed from the file.

What the downloadable file contains

The published CSV carries these columns, in this order. NPISignal imports all of them.

DOBDate of birth, for individuals. The strongest identity field the file carries.
ADDRESS, CITY, STATE, ZIPAddress on file at the time of exclusion.
EXCLTYPEThe statutory authority for the exclusion, e.g. '1128b4'.
EXCLDATEThe date the exclusion took effect.
REINDATEReinstatement date, where one applies.
WAIVERDATE, WVRSTATEWaiver information, where a state has granted one.

How to download the LEIE

OIG publishes the complete file monthly at oig.hhs.gov/exclusions/exclusions_list.asp. It is free, it needs no registration, and the current release is always at the same URL. OIG also publishes monthly supplements listing additions and reinstatements.

NPISignal imports the full monthly file rather than applying the supplements incrementally. A full replacement means the copy served here is always exactly one published release — there is no accumulated state that could quietly drift out of agreement with OIG’s own file.

The file cannot resolve every identity

The downloadable LEIE contains no Social Security number and no employer identification number. Where two people share a name and a date of birth, the public data cannot tell them apart. OIG operates a separate verification process for exactly that situation, and it is the authoritative route — NPISignal does not automate it and will not claim to have used it.
